How to Request Your New York CTLE Completion Documentation
If you completed an AOE professional learning experience (NOW Conference or PRO Learning) and need documentation to apply those hours toward New York State CTLE requirements, complete this form to request your CTLE Certificate of Completion.
AOE will verify your completed learning experience and provide the documentation needed for your NYSED CTLE records.
What do I need to complete this request?
Please have the following information available:
To complete this request, you will need:
1. Your AOE account information
2. The Certificate of Completion for the learning experience you participated in
3. Your personal information as required by New York State CTLE regulations
How This Process Works
Step 1: Complete this request form
Provide the required information about yourself and the AOE learning experience you completed.
Step 2: AOE verifies your completion
AOE will confirm that you completed the learning experience and CTLE hours associated with your activity.
Step 3: Receive your CTLE documentation
AOE will provide your completed CTLE Certificate of Completion. You may use this documentation to submit your hours to NYSED or retain it with your professional records.
Processing Expectations
Your request will be reviewed after submission.
Expected turnaround:
AOE will process requests within 20 business days after receiving complete information.
If additional information is needed, AOE will contact you using the email address provided.

Why do we collect this information?
New York State requires approved CTLE sponsors to maintain specific participant records for completed CTLE activities. The information collected through the New York CTLE Completion Form allows AOE to create and maintain your required CTLE documentation.
Your information will be securely stored and retained in accordance with New York State CTLE record-keeping requirements.
